CASE SCENARIO..
 
I am Sanjana MP , 3rd BDS student. This is an online elog book to discuss our patients health data after taking his consent.  This also reflects my patients centered online learning portfolio. 

CASE HISTORY 
Patient details:
A 57 years old male , occupation- farmer 

Chief complaint:
Fever from  10days 
Less urine output from 10days
Swellings hands and legs upto knee since 6months 

History of present illness:
Patient is asymptomatic 6months ago 
-Urine - decreased output from 10days, burning sensation during urinating. 
'-Fever -from 10days, low grade, intermittent associated with chills relieved on medication 
-Swelling-since 6months,  upper and lower limbs , pitting edema uptill knee
-no abdominal pain or chest pain

History of past illness:
Patient has hypertension since 5 years 
Diabetes mellitus since 10 years 
Patient got seizure attack once 2years back 
Coronary artery disease since 2 years 
Patient has no history of TB or asthma 

Family history:
No history of similar complaints in the family 

Personal history :
Diet- vegetarian 
Appetite-loss of appetite 
Sleep- improper sleep
Bowel - normal
Micturition - abnormal
Habits- smoking stopped 2years back
            - alcohol stopped 2years back

General examination :
Patient is conscious and not cooperative
Pallor - no
Icterus-no
Clubbing- no
Cyanosis- no
Lymphadenopathy- no
Edema - yes, pitting type. 

Q/A 
1. Does burning sensation during urination indicates any severe kidney problem?
2. Is the  edema completely curable?
